COVID19 Safety Screening at the Youth Centre!
Thank you for your patience, understanding, and consideration of others.
Any person accessing the facility (e.g., staff, volunteers, participants, parents, etc.) should perform a COVID-19 self-assessment before arriving at the facility.
Upon completion, please send the results PDF to us via facebook msg or email@example.com
Anyone arriving at the centre who has not completed the self-assessment will be screened using the same process at the entrance. This assessment can also be done on your phone and shown to us!
Please help us in making sure your child's membership form is up to date especially contact information!
Any person with a symptom(s) must not enter the facility, should self-isolate and consider getting tested at a local assessment centre.
